Who is Us2 Behavioral Health Care?
Us2 Behavioral Health Care is a non-profit, outpatient mental health facility founded in 2019. The outpatient clinic, located in Appleton, provides counseling and assessments to children and adults.
How can my child become involved in school-based therapy?
If you are interested in getting counseling services for your child, please contact your school counselor or contact Us2 at 920-903-1060 and let them know you are a parent/guardian of a student at Neenah Joint School District. Students are scheduled at a school in the order they are received. If there is a safety risk, students may not be appropriate for school-based therapy right away. Us2 will screen the appropriateness of referrals. Availability of treatment can change quickly, however, if someone would rather not be put on a wait list, other options, including seeing an Us2 therapist at their office will be offered.
What is the intake process like?
An intake appointment is time for the therapist to meet your child, begin forming a relationship, and get to know them. During intake, the therapist will collect information from you and your child about presenting concerns, current symptoms/behaviors, medical history, and family/social history. This helps the therapist to gain insight into what is going on in your child’s life from your child and your perspective. Your therapist will then work with you to develop a treatment plan which will set goals and guide future therapeutic sessions.
What paperwork will be required before my child begins therapy? Do I get it from school or from Us2?
Us2 has forms that you will review and sign prior to your child’s first appointment. These must be signed by a parent or legal guardian. The forms include our service agreement, forms related to you and your child’s rights as a client of Us2, and release of information to communicate with your child’s school. Additionally, if sent electronically through TherapyNotes portal, you can provide your therapist with concerns in advance through the patient history form. If you cannot fill these out online through the TherapyNotes portal, you can pick up a copy from the therapist at school, or ask Us2’s administrative team to mail you copies for your child to bring to their appointment. The Us2 number is 920-903-1060.
How much does therapy cost?
Therapy is usually covered by most commercial and medicaid insurance companies. Copays and deductibles may be owed, and are determined by each individual health plan. Us2 also offers self-pay rates for those who are uninsured or for insurance plans that Us2 is not in-network with. When scheduling initial intake appointments, families can request a financial review to determine self-pay rates.
What insurance companies is Us2 in network with?
Us 2 Behavioral Health Care is continually adding insurances so this is not an exhaustive list. Please call Us2 at 920-903-1060 to see if your insurance is in their coverage. Some of the insurances that are covered include: United Health Care, UMR, Network Health, Humana, Anthem Blue Cross Blue Shield, Common Group, WPS, Arise, Alliance, and Cigna. Us 2 Behavioral Health takes medicaid plans as well; HMO’s include Molina, Children’s Community Plan, Cenpatico, Network Health, Blue Cross Blue Shield, Optum, United Health Care, and straight medicaid. Us 2 is always working to contract with more insurance companies, so this list may change. Call Us2 at 920-903-1060 to see if your insurance plan has been added. If not, Us2 can discuss alternative options with you.
What is Us2’s cancellation/attendance policy?
Us2 expects parents or guardians to give a 24 hour notice when they need to cancel or reschedule an existing appointment. If received in less than 24 hours, that is considered a late cancellation. Clients can be terminated from services if they have three no-show appointments and/or three late cancellations (less than 24 hour notice) within a two month period (or a combination of the two). To notify us of your child missing therapy, you can call or text 920-903-1060.
Will it count as an unexcused cancellation for therapy if my child is sick from school on the day they have an appointment scheduled with the therapist?
Us2 prefers to have a 24 hour notice of cancelling any appointment. Sickness is an approved absence, so letting us know as soon as possible is appreciated. To notify us of your child missing therapy, you can call or text 920-903-1060. However, if your child decides they “don’t want to go” right before their session this would be an unexcused absence from therapy. In that case, there may be a charge for services.
How do I contact the therapist if I want them to know something important before they see my child next?
The best way to reach your therapist is to call them at 920-903-1060 and leave a voicemail. Therapists are able to access their voicemails off site while at the schools and can return calls between appointments. Please allow 24 hours for your call to be returned.
Can my child continue to see their therapist during the summer or school breaks?
Yes, your children can continue working with their therapist through our outpatient clinic or via telehealth during the summer or winter breaks. Your child may resume services at school once the school year starts.
Can other family members see therapists at Us 2 Behavioral Health Care?
Yes! Us2 has therapists that serve clients ages 4+ years old, for a variety of concerns, and in a variety of languages. We can do both in-person and telehealth sessions. Some of our therapists are bilingual and speak Hmong and Spanish. You can learn more about our therapists by reading their bios at https://us2behavioralhealthcare.com/counseling/, or calling our office at 920-903-1060.
What types of issues can my child be seen for by an Us 2 therapist?

However, most have experience in concerns such as anxiety, depression, impulse control/regulation, self harm, suicidality, bullying, traumatic experiences, parent/child issues, family/life transitions and changes, LGBTQIA+ identity, and grief/loss.
