Online Nomination Form

Instructions:  Please complete as much of the nomination form as possible. It is not required to answer every question, but the more information provided will greatly assist the committee in the selection process. Additional information such as articles or letters of recommendation may be e-mailed to jstrick@neenah.k12.wi.us or brought to the Administration Building at 410 S. Commercial St.

Qualifications: If the candidate is a student/graduate, then the candidate must have graduated from Neenah High School a minimum of twenty (20) years prior to his/her nomination.  If the candidate is a coach, administrator, teacher, booster or volunteer, then the candidate must have retired or no longer work/volunteer for the NJSD a minimum of five (5) years prior to his/her nomination.  If circumstances warrant, the aforementioned waiting periods may be waived by NJSD Hall of Fame Selection Committee.

All candidates must exemplify the attitude and code of conduct that is synonymous with the Neenah Joint School District.
