Attention Bus Riders: At the right is a Rider Certifcation form which is required for bus riders. Please fill out the form and return it to the Shattuck Office before September 9, 2011.

Also to the right is a Bus Riders Handbook of Rules as a reference.

Updated information is located to the right of this message. The forms are optional items to bring to registration. The parent handbook is revised and posted.

NOTE: Student Demographic Information forms are not shown as a form to print. The Student Demographic form will be available along with class schedules at registration.

Follow this link to athletic forms which can be printed, filled out and returned to the coaches.

Note: If a student had a physical examination last year after April 1, 2010 just the Alternate Year Physical card is required along with the Permission to Participate, Insurance Waiver and Athletic Emergency Information forms.

Student Fees
 
Required: $45 General School Fee and the total amount of any outstanding fines from the previous years.
(Reduced Fee is $18.)

Optional: $30 per activity - fee for participation in athletics, debate, drama and forensics. Students will be charged a maximum of $60 if participating in two or more of these types of activities:

        •$120 maximum per family

        •$320 maximum per family with students at NHS & Middle School.

        •$20 yearbook subscription order (before 12/23/11).

        •$13 heart rate monitor strap

        •$75 instrumental rental fee

*Optional fees will not be accepted until General School Fees and outstanding fines have been paid.

* Fee amounts are subject to change.

* Note:  We request that fees be paid by check and made payable to Shattuck Middle School for the correct amount. If sending a check with your child, please include a note indicating what fees are to be paid. Remember that outstanding fines must also be paid at this time. Parents and students will be provided with a receipt for fees paid.

In those situations where a financial hardship exists, students may be eligible for a reduction or waiver of school fees utilizing the same criteria established for free or reduced meals if the agreement form is signed.
