0 = GREAT!
You have obviously made a decision to not have drugs & alcohol be a negative part of your life. You are clearly making some healthy choices. Remember that you are always responsible for the choices you make. Keep it up!
1 to 3 = CONCERN.
Dependent upon which statements were marked yes, having 1, 2 or 3 marked is an indicator of the start of a drug problem. This is the stage when the drug starts slowly taking over. At this stage, seeking help early is the best solution to saving your future. Talking to any of our school counselors or with SAP is recommended. Check out our NJSD SAP website for more information.
4 to 6 = GRAVE CONCERN.
If you are able to mark yes to 4 to 6, you are well on your way to addiction, if not there already. Seeking help becomes more difficult because at this stage addicts rarely see their use as a problem. This would be a great time to speak to a counselor at school or with SAP. 7 or more = ADDICTION.
To indicate yes on 7 or more of these statements clearly means a serious problem with drugs and/or alcohol. Generally speaking, when a person reaches this point, a more intensive counseling service is required. Your counselor and/or SAP can get you there but the first step is asking for the help.
