FALL 2010 - Start of Practice & Equipment Checkout Dates for HIGH SCHOOL and MIDDLE SCHOOLS Download and read the entire document here. All Athletes must present the following to their coach the first day of practice
:
- Authorization for disclosure of Health Information
- Emergency Card
- Proof of a Physical and/or Alternate card
- An Insurance waiver
*** Athletes will not be allowed to practice until all paper work is completed and turned in. All Athletes must attend a mandatory Code of Conduct meeting before their season starts. ...more (pdf)
Activities Code of Conduct Meetings The meetings will be on Thursday, August 19 at 7 p.m. and Monday, August 23 at 5:30 p.m. and 7 p.m. Every student planning to participate in athletics or activities MUST attend with at least one parent.
